Sunnyvale Imaging is here to help with any questions or concerns you may have prior to your exam. We are committed in providing un compromised care. In order to prepare you for your visit we have provided online patient resources.
Insurance Information
Sunnyvale imaging center is contracted with major Health insurance carriers, for your convenience we also check the benefits on your current plan.
Please bring a valid picture ID and current insurance information this includes Medicare.
If this is an auto or personal injury case, please bring in your auto med-pay, your attorney’s information, along with your valid picture ID and general insurance information.
If the is a worker’s compensation case, please bring your claim number along with case adjuster’s name and contact information.
Self Pay
We accept Visa, MasterCard, and Personal Check.
Exam Preparation
There is no special preparation for MRI, X- Ray, and DEXA exams.
There are no restrictions on your diet, so feel free to eat and drink within your normal diet.
If possible, wear comfortable clothing, and remove all metal including jewelry. We also recommend leaving all valuables in a safe place; however, we do provide secure lockers for your personal belongings. When needed, gowns are provided.
X-Ray and DEXA:
You have a choice between making an appointment or walk-in. We perform X-Rays and DEXA exams between 8:00-11:45 a.m. and 1:00-4:45 p.m.
For MRI exams:
All exams are scheduled; therefore, prior to your exam, please allow 30 min to complete your registration.
Please bring the following to your appointment:
- Health Insurance Card
- Physician Order
- Photo ID
